Over 50s in rural areas missing out on modern healthcare ...

A study by researchers at the University of Stirling shows that due to a lack of knowledge and skills, older patients in rural areas are being denied the benefits that modern communication technologies such as text, Web and video can bring in health care delivery.

The survey, jointly led by Professor Stephen Leslie, a cardiologist at Raigmore Hospital and Honorary Professor in the School of Nursing, Midwifery and Health at the University of Stirling, and Dr Amir Hussain at Stirling?s Cognitive Signal Image Processing Research (COSIPRA) Laboratory in the School of Natural Sciences, questioned cardiology patients at Raigmore Hospital, in Inverness, about their use of digital technology.

The findings show that young patients are more likely to use the Web, mobile phones and texting than patients over the age of 50. However, the majority of patients, regardless of age, had access to email.

NHS Highland is looking at the use of digital technology to provide a faster and more cost effective method of healthcare provision, as more than 30 per cent of Raigmore Hospital?s cardiology patients live in remote locations and have to travel more than one hour to attend appointments.

Studies have shown that with support, new technologies can be successfully used amongst the over 50s for clinical care. These include methods such as ?tele-assistance?, where a patient can receive ?telemedicine? by speaking to a physician through video conferencing, with support through email and Web communication.

The survey, which also assessed attitudes towards the use of technology in care, will be used to guide future redesign of communication with NHS cardiology patients.

Professor Leslie said: ?There may be several potential barriers to the widespread implementation of communication technology to a general cardiology clinic population, particularly in remote and rural areas.

?Increased use of communication technologies have the potential to improve patient care. Implementation of these can be difficult and patient skills and confidence with them will vary.

?NHS policy makers, clinicians and managers should take note of these findings when attempting service redesign as this is a very important factor to consider.?

Dr Hussain added: ?The potential benefits for the delivery of healthcare services by greater use of technology are considerable.

?Traditional face-to-face medical outpatient clinics are a common way of assessing and monitoring patients and have been in place for many years, but many patients are seen in the clinic with little or no positive outcome in terms of treatment decisions, with some patients being seen as a matter of routine. Traditional doctor-led clinics may not be an efficient model of care.

?The benefits of more frequent or convenient communication with a doctor appeared to be more important than potential subtle reductions in quality of consultation. The patients were less affected by the way in which doctors interacted with them and more concerned about whether this interaction occurred at all.?

The research paper is being published in leading journal Telemedicine and e-health in April.

Heading into Oscar night, a collection of safe nominations and clear favorites has led to a distinct lack of drama. For the past few months, no other film has led the awards conversation quite like "The Artist." An unlikely favorite, yes, but Michel Hazanavicius' ode to the silent-film era moved to the front sometime before the Golden Globes and stayed there.

In the days leading up to the ceremony, the biggest questions aren't who will win, but whether there will be any major surprises at all. Most Oscar analysts have locked their picks in by now, and the apparent answer is a resounding "probably not."

Is "The Artist" unstoppable? Who would we like to see win? Here are our picks for Best Picture:

Who Will Win: "The Artist." After stealing the early lead as favorite from "The Descendants," nothing has stood in the way of "The Artist." Even the most British movie of all time, "Tinker Tailor Soldier Spy," couldn't beat the silent film at its own awards show, the BAFTAs. Oscar gold for Hazanavicius' feel-good love letter to cinema is essentially a foregone conclusion at this juncture. If you're looking for an Oscar-night surprise, look elsewhere.

Who Should Win: "The Tree of Life." Terrence Malick's all-encompassing look at life on this little blue ball is not only the most ambitious film of the nine nominees, but the only one that took any risks at all. Yes, "The Artist" goes without sound for most of its runtime, but the silence rarely goes any deeper than gimmick and relies on an already-established style of filmmaking. Malick, instead, went in an entirely different direction and made a movie on his terms, using his own unique sensibilities. Complaints about pretension are valid and should be acknowledged, but when the rest of the field plays out like safe bets, you have to respect that Malick is swinging for the fences.

George Huguely V: Guilty of Second-Degree Murder in Yeardley Love Slaying


George Huguely V, a former University of Virginia lacrosse player, has been found guilty of second-degree murder in the slaying of his ex-girlfriend Yeardley Love.

A Maryland jury convicted him late Wednesday.

The UVA lacrosse murder case began in May 2010 and gained national attention, after Love, a popular, all-American athlete, was found dead at the school.

There were no obvious physical wounds. Huguely, 24, admitted to having an altercation with his ex, but his lawyer denied her death was intentional.

Huguely pleaded not guilty to six charges, including first-degree murder and felony murder in commission of a robbery, professing his innocence ... mostly.

His defense attorney Francis Lawrence said during closing arguments:

"Yes, George contributed to her death. But no, he didn't kill her ... he left there with her alive, and that is not in dispute. There was no intentional killing."

"She wasn't dead when he left."

After deliberating for nine hours, the jury decided Huguely was guilty of second degree murder, which can hold a sentence of anywhere from 5-40 years in jail.

Huguely was found not guilty on the most serious charge of first-degree murder, but he was also convicted of grand larceny stemming from the same event.

Public still opposes auto bailout, poll finds

While the U.S. auto industry may be ?back,? as President Barack Obama recently declared during his State-of-the-Union address, that hasn?t changed the fact that a majority of Americans remain opposed to the 2008 ? 2009 bailout of Detroit.

A new Gallup poll finds 51 percent of those surveyed still disapprove of the $85 billion rescue effort, with only 44 percent?saying they approve.? And the figure is even more lopsided when party affiliation is consideredD.? The poll, conducted for the public radio program Marketplace, found 73 percent?of Republicans opposed ? though 63?percent?of Democrats were supportive of the bailout.

Americans keeping cars longer than ever

Though the results are still in negative territory, overall, that?s still a significant improvement from 2009, when various polls?showed that the vast majority of Americans, regardless of political persuasion, were unhappy with the use of taxpayer money to save General Motors and Chrysler.

The numbers have been slowly moving back into positive territory, various surveys have found, especially as the two once-bankrupt makers have started posting solid earnings again ? and have been adding thousands of new jobs.? On the other hand, reports that the U.S. Treasury could still lose perhaps $20 billion on the bailout ? especially if GM?s stock price doesn?t rebound ? continues to throw up a red flag.

And the GOP presidential contenders all but trip over one another in debates and stump speeches, to turn the bailout into a liability for President Barack Obama.

During a Wednesday night Republican debate in Phoenix, former Massachusetts Governor Mitt Romney said, ?I think it was 2008, President Bush was still in office. And the three chief executive officers of the three major auto companies got in their private planes and flew to Washington, and said: please write us a check. I think they wanted $50 billion. And I wrote an op-ed in the paper and I said: absolutely not. Don?t write a check for $50 billion.?

While the bailout is likely to remain a subject of contention for some time, the new Gallup poll isn?t entirely negative for the auto industry overall.

?There was a time when the auto industry and our positive/negative image ratings was down at minus 35; that was in 2009,? said Gallup editor-in-chief Frank Newport. ?In fact, it was second only to the oil and gas industry, which interestingly ? perennially ? pulled up the bottom on that.?

Now, he noted, the auto industry is back in positive territory, with a plus-10 rating.? That?s still well below the image of the computer industry but well ahead of oil and gas ? and the federal government, which now ranks dead last.

The debate about the potential severity of an outbreak of airborne H5N1 influenza in humans needs to move on from speculation and focus instead on how we can safely continue H5N1 research and share the results among researchers, according to a commentary to be published in mBio, the online open-access journal of the American Society for Microbiology, on Friday, February 24.

H5N1 influenza has been at the center of heated discussions in science and policy circles since the U.S. National Science Advisory Board for Biosecurity (NSABB) asked the authors of two recent H5N1 investigations and the scientific journals that planned to publish the studies to withhold crucial details of the research in the interest of biosecurity.

In the mBio commentary, Michael Osterholm* and Nicholas Kelley, of the Center for Infectious Disease Research and Policy at the University of Minnesota, present their case that H5N1 is a very dangerous virus, based on their analysis of published studies of the seroepidemiology of H5N1 in humans. H5N1 flu infections have exceedingly high mortality, they say, and current vaccines and antiviral drugs will not pull us out of a global H5N1 pandemic. "We believe that the assertion that the case-fatality rate of H5N1 influenza in humans may be overestimated is based on a flawed data analysis," Osterholm said.

Analysis of reports of H1N1 seroprevalence that include data from the 1997 Hong Kong outbreak as well as data from 2004 to date will give a misleading impression because the 1997 outbreak was a very different "biologic event" that is recognized as such by the WHO, because the 1997 H1N1 virus has a significantly different genotype from that of later H5N1 viruses. This is why the WHO does not include the Hong Kong H1N1 virus data in any analysis of H5N1 transmission, and the 1997 Hong Kong virus is not recommended for inclusion in H5N1 vaccines, Osterholm explained.

Seroepidemiologic studies that have examined the exposure of various groups of people to H5N1 viruses only from 2004 onward indicate that only a small segment of the population has ever been exposed to H5N1, and that among those that have been exposed, many become seriously ill or die.

"The available seroepidemiologic data for human H5N1 infection support the current WHO reported case-fatality rates of 30% to 80%," Osterholm says. In the event of an H5N1 pandemic, they point out, if the virus is even one tenth or one twentieth as virulent as has been documented in these smaller outbreaks, the resulting fatality rate would be worse than in the 1918 pandemic, in which 2% of infected individuals died.

Vaccines will not head off an H5N1 pandemic either, the authors say, since the time required to develop and manufacture an influenza vaccine specific to new outbreak strain has resulted in "too little, too late" vaccine responses for the 1957, 1968, and 2009 influenza pandemics, and not much in the process has changed since 2009.

"The technology behind our current influenza vaccines is simply not sufficient to address the complex challenges associated with an influenza pandemic in the 21st century," Osterholm and Kelley say.

This is the heart of the matter, they say: there has been enough discussion about how severe an H5N1 pandemic might be. Moving forward, the current controversy has provided a valuable opportunity for scientists and public policy experts to discuss influenza research and preparedness and create "a roadmap for the future." The discussion among scientists and policy makers needs to move on from whether H5N1 poses a serious international threat - as it clearly does - and begin discussing how we can prevent these viruses from escaping labs and how scientists can share their flu-related results with those who have a need to know.

There are critical questions that need to be answered, the authors say. For instance, how can scientists conduct virus-transmission studies in mammals safely and how can scientists share research methods and results with those who have a need to know? We also need to come to agreement on how to ensure that strains of H5N1 viruses created in the lab don't escape those controlled environments, the authors say. And new, more effective vaccine technologies are needed that can enable substantially faster production. Resolving these issues could allow H5N1 research and preparedness to serve as a springboard for solving similar problems with existing or emerging pathogens.

Single atom transistor gets precise position on chip

The basic unit of matter could become the basic unit of computing. A lone atom of phosphorus embedded in a sheet of silicon has been made to act as a transistor.

It is not the first single-atom transistor, but it can be much more precisely positioned than its predecessors, potentially making it a lot more useful.

"It's an absolutely fantastic piece of engineering," says physicist Bruce Kane at the University of Maryland, who was not involved in the work.

Elaborate production methods would initially prevent single-atom phosphorus transistors from being a worthwhile addition to traditional computers, but they may be necessary one day. The devices could also find an application in futuristic, super-speedy quantum computers.

A transistor is essentially a lump of conducting material sitting between two electrodes that acts as a switch. A pulse of voltage is supplied by a further electrode,"opening" the switch and allowing current to flow through the transistor.

Wiggling atom

Combining transistors on a chip produces logic circuits that can carry out computations. A goal shared by computer chip makers is to keep shrinking the transistor: squeeze ever more onto a single chip and you increase its computational power.

To dictate the exact position of their single atom, Michelle Simmons at the University of New South Wales, Australia, and colleagues started by covering a silicon sheet with a layer of hydrogen. Then they used the tip of a scanning tunnelling microscope to remove hydrogen atoms according to a precise pattern. They exposed two perpendicular pairs of exposed silicon strips plus a tiny rectangle made of just six silicon atoms that sat at the junction between these strips (see diagram, right).

Adding phosphine gas (PH3) and heating caused phosphorus atoms, which are conducting, to bind to these exposed areas of silicon. In the case of the rectangle only one atom inserted itself into the silicon network.

The result was four phosphorus electrodes and a single phosphorus atom.

Boutique operation

One pair of electrodes was separated by a 108-nanometre gap. Creating a voltage between them allowed current to flow between the two perpendicular electrodes ? separated from each other by just 20 nanometres, through the single phosphorus atom, which acted as a transistor.

Kane points out that the atomic transistor works at temperatures below 1 kelvin and that fabrication is difficult. "It's a very slow, boutique operation to make one of these," he says.

Simmons agrees, but counters that the traditional computer makers may be forced to adopt this technology if they want to make ever smaller chips. "This is one of the only techniques that allows you to make single atom devices," she says.

Physicist Jeremy Levy of the University of Pittsburgh in Pennsylvania reckons the future of single atom transistors lies in quantum computers. The spin of the electrons in isolated phosphorus atoms could serve as qubits, the quantum equivalent of the bits in today's computers. Controlling the interaction between qubits requires knowing the exact location of each one. Now that the location of individual atoms can be controlled, the next challenge is to link two of these transistors, Levy says.

Reporters needled New York Attorney General Eric Schneiderman at a Crain?s breakfast forum this morning ?where the AG announced his proposal for non-profit reform, saying that his appointment to the joint investigation unit has made it seem like he was ?co-opted by the administration,? and would be less aggressive with banks as a result. The AG defended himself and the administration, saying that the immunities granted for banks in the recently signed mortgage settlement were narrow, and that he intends to pursue the matters he is legally allowed to to the full extent of the law.

President Barack Obama announced the formation of the new taskforce to investigate home loans and mortgage-backed securities fraud in his state of the union address.

On the topic of the Mortgage Electronic Registry System lawsuit, Schneiderman said ?it?s a challenging situation,? that resulted from deregulation ?to the point of recklessness.? He added that property law was ?absolutely fundamental to our way of life.??Schneiderman?s office filed the suit two weeks ago against major banks, alleging ?deceptive and fraudulent? practices.

He also said that although he and the business community have butted heads in the past, he is an advocate for the private sector.??My role as a regulator is to catch the bad guys,? he quipped, ?but also to help the good guys? do business. ?Our financial markets are models to the world, and we should keep them that way.?

The breakfast was focused on Schneiderman?s proposed legislative changes, which would make it easier for non-profit organizations to form and operate in New York City, while also tightening oversight. He noted that one out of every five jobs in New York City is in the non-profit sector, which accounted for $200 billion in revenue for the state last year.

The AG also announced a new program, called New York on Board, which is intended to encourage dialogue between the business and non-profit communities, and help business leaders take part in non-profit leadership. He said that the commercial brokerage Cushman & Wakefield had already signed on as a participant.

Cushman & Wakefield could not immediately be reached for comment.

?I knew something was wrong, but I was like, ?OK, I?ll keep on playing as long as I can play,? and I was able to get through it all,? Slauson said Thursday, according to Jenny Vrentas of the Newark Star-Ledger.? ?I felt like I had a fairly successful year personally with it.? So I?m really excited about next year without having pain.?

Slauson had full-blown surgery after the season ended.? He believes he suffered the injury in November 2010 against the Patriots.

But he claims he never got the injury checked out.? Presumably, that would insulate the Jets from scrutiny; if they never knew Slauson was hurt, they would have had no reason to disclose the injury.

Still, the Jets knew in Week 10 that Slauson had a neck injury that didn?t require full-blown surgery; he was listed as probable that week.? And the Jets knew in Week 11 that Slauson had an ankle injury that didn?t require full-blown surgery; he was again listed as probable.? And the Jets didn?t know in Weeks 12 through 15 that Slauson had a knee injury that didn?t require full-blown surgery; he was listed as probable for each of those four games.

Slauson never was listen on the injury report as having anything wrong with his labrum, rotator cuff, bicep, or shoulder.

So, yes, it seems more than a little implausible that Slauson could have, for more than a year, a torn labrum, a torn rotator cuff, and a torn biceps and that neither he nor the team knew anything about these conditions.

It?ll be interesting to see whether the league office takes the Jets? and Slauson?s word for this one, or whether the league asks to inspect any relevant training-room records that would reveal whether or not Slauson received treatment.

Loved 'The Artist'? See LA's old silent film spots (AP)

LOS ANGELES ? From around 1910 to the late 1920s, the silent film industry dominated Los Angeles. The movies were filmed everywhere, from Hollywood to bustling downtown to what was then a nearly barren valley area, on the other side of the Hollywood Hills. Without permits, unions or worries about sound, filmmakers could just grab a camera and shoot scenes on the spot, transforming various L.A. locales into any place the script called for. Hollywood was truly the Wild West, infinitely more accessible than now.

"The Artist," a Golden Globe winner and Oscar contender that hearkens back to the lost art of telling a story in black and white, without talking, has renewed interest in that early genre. Fortunately, many of the locations where exteriors were filmed during the silent film era still exist today, and you can find them hidden around the city like historic gems.

"Southern California was perfectly situated" as a backdrop for all types of movie settings, said film historian John Bengtson, author of the books "Silent Traces: Discovering Early Hollywood Through the Films of Charlie Chaplin," "Silent Echoes: Discovering Early Hollywood Through the Films of Buster Keaton" and "Silent Visions: Discovering Early Hollywood and New York Through the Films of Harold Lloyd."

"There was a diversity of geological features, the beach, desert," Bengtson said. "There were rough terrains for the Westerns. There were mountains. There were lakes. Downtown Los Angeles was a thriving city, so you got your urban shots. It was just ideal."

Bengtson started researching then-and-now locations from scenes in the films of silent comedy stars Buster Keaton, Charlie Chaplin and Harold Lloyd more than 15 years ago, without the help of the Internet. He's since identified dozens of locations, and has conducted various silent film walking tours.

One famous image from that era that lives on shows Lloyd clumsily climbing up the side of a building in downtown Los Angeles to escape a police officer in the 1923 romantic comedy "Safety Last!" Lloyd, in his signature straw hat and round horn-rimmed glasses, then grasps onto a large clock on the building. He hangs on for dear life with traffic rushing far below. The long, nail-biting scene has been referenced in multiple movies, from "Back to the Future" to one of this year's Oscar best picture nominees, Martin Scorsese's kid adventure "Hugo."

And the tall building in downtown L.A. where Lloyd shot that famed clock scene still stands, at 908 S. Broadway. The clock, constructed specifically for the movie, doesn't. The beautiful Orpheum Theatre, which didn't open until three years after the movie was shot, in 1926, is next door.

A building facade was actually built on the 908 S. Broadway building's roof, along with a camera tower to film the set, in order to create the illusion of steep height, keeping the building's roof out of frame, but with actual views of the street below, said Bengtson. There's a palpable sense of anxiety in viewing the movie, with Lloyd avoiding dogs and wayward wooden planks plunging out of windows as he scrambles up. Lloyd filmed many projects downtown, said Bengtson.

"They didn't have CGI (computer generated imagery) then. They did have glass paintings," said Randy Haberkamp, the Academy of Motion Picture Arts and Sciences' director of educational programming. "They would paint out part of the city. You would see a famous hill in the Los Angeles area, and you would say, `Where was that house there?' They could create a sense of depth and danger. The artists of that era were so clever."

Kansas-born actor and director Keaton, with his melancholy good looks, sad eyes, dark hair and deadpan expression, is best known for silent films from the late 1920s like "Steamboat Bill Jr." and "The General," set in the American Civil War. Keaton filmed many short and full-length comedies in Hollywood, downtown, west of downtown and in the beachside Venice and Santa Monica areas near the Pacific Ocean.

In his 1921 short film "Hard Luck," a suicidal and broke Keaton gets into all sorts of shenanigans, including being chased by a bear and unsuccessfully trying to hang himself. At one point, eluding police, he poses as a statue next to an imposing bronze statue of General Harrison Gray Otis, longtime owner and publisher of the Los Angeles Times, in MacArthur Park, 2230 W. Sixth St., by Wilshire and Park View Street, west of downtown.

The statue still stands today, pointing in the direction of Otis' old house. At that time, there wasn't a great deal of public art in L.A., said Bengtson, so Keaton creatively used the sculpture as a backdrop. Physical comedy worked seamlessly with soundless cinema.

Comedy duo Stan Laurel and Oliver Hardy, who started off in silent film, also utilized broad swaths of L.A., including the now-hipster enclave of Silver Lake on the eastern side of town.

In one of their best-known talkie films, "The Music Box" (1932), the pair attempts to push a piano up a multi-tiered outdoor staircase, only to have it fly down the stairs after them. The staircase, named the "Music Box Steps" in the `90s, is next to 923-935 Vendome St., near the intersection of Del Monte, in a quiet, residential Silver Lake area lined with shrubs, flanked on either side by houses and marked by a black commemorative plaque on one of the stairs.

It turns out the staircase was first used by Laurel and Hardy in their then-popular but now lost 1927 silent film "Hats Off" in a scene involving the two hauling up a huge, round washing machine. According to stills from the movie, only overgrown fields surrounded the stairs at the time.

Actor, writer and director Chaplin, iconic as a vagrant, big-hearted character known as the Tramp in many of his silent films, complete with bushy short mustache and black bowler hat, filmed all around L.A. before settling into his Charlie Chaplin Studios, 1416 N. La Brea Ave., in the heart of Hollywood. The structure is now the Jim Henson Company Lot, with a statue of Henson creation Kermit the Frog dressed as Chaplin in his recognizable hat and jacket above the main gate. Concrete footprints of Chaplin are also there.

Chaplin's studio opened in 1918, and most of his known films were mainly shot on the premises, including feature-length hit 1921 silent dramedy "The Kid" and the 1925 romantic comedy "The Gold Rush." Chaplin's 1919 silent short film "A Day's Pleasure" starts off with Chaplin and his fictional wife and kids trying to take off in their clunky car outside the back corner of Chaplin's studio.

Several outdoor scenes for "The Kid" were shot at packed downtown L.A. Mexican marketplace Olvera Street, years before the alley was converted into a tourist attraction, across from the current L.A. train hub Union Station, 800 N. Alameda St. A particularly poignant, emotional scene in "The Kid," when Chaplin reunites with his scrappy adopted son, played by Jackie Coogan, takes place outside an old brick building structure, still in place at Olvera Street.

The synchronicity of emotion, environment, celebrity and public access in L.A. made silent films powerful for their era, before talking pictures added layers of complication, both technically and professionally, for actors and filmmakers. Silent movies had a worldwide, universal appeal, even if only filmed within what was then the small town scope of Hollywood.

"Because you didn't hear the stars speak, their faces, their pantomime, was important. There was an automatic romance to that too. You imagined what your leading lady or man sounded like," said Haberkamp. "Sometimes, when it comes to talking, sometimes less is more. I think people who love silent films really appreciate body language and other things that are communicated. There's a deeper human understanding that you can get."
